10.Certificate of No Accountability of Electric Bill(MOELCI-II Finace Department for Ozamiz City and MOELCI-II Sub-office for Sub-offices.

11.Photocopy of Residence Certificate(CEDULA) or Any Valid ID.

12.Picture 1x1 - pcs each (husband and wife).

13.Attendance of Orientation Seminar (9:00AM)

14.Membership fee - Php 5.00 (New Member).

15.Signed Membership Form (New Member).

16.Signed MOELCI-II Electric Service Contract.

17.Signed Waiver (Consumer 30 meters away from the nearest pole).

Note: All residential applicants are given free KWH Meter,30 meters of Service Drop Wire and installation of meter.
